DIANNE C. BRALEY

Dianne C. Braley is a Registered Nurse with over 24 years of experience across a wide range of clinical settings. She began her career in post-surgical care at one of Boston’s leading hospitals before moving into acute care and ICU, where she developed a strong foundation in high-acuity patient care.

Her experience extends into home care, case management, and care coordination, where she has worked with one of the largest home care agencies in the state. She has built strong relationships with Elder Services of the North Shore and multiple community-based organizations, supporting patients with complex medical and social needs.

Dianne has also worked as a private nurse on Martha’s Vineyard, providing individualized care to high-profile clients. She is a certified Alzheimer’s coach, nurse nutritionist, and former Sexual Assault Nurse Examiner (SANE). She is also an OASIS expert with extensive experience in Medicare-based home care documentation and reimbursement, as well as an ICD-10 certified clinical documentation specialist.

After decades in healthcare, Dianne saw how often patients felt lost in the system. This work is her response — helping patients and families feel informed, prepared, and confident in their care.

KERI POMEROY

She started her career at just 16 years old as a CNA, progressing to a PCA/Medical Assistant in Neurology at one of Boston’s largest Hospitals.

She has since built an extensive background in home care, case management, and patient advocacy. Keri has worked with leading home health agencies across the state, serving as a nurse manager and playing a key role in building and supporting agencies from the ground up, including involvement in franchise development.
Keri has also developed and led internship programs to train new nurses entering home health, helping prepare them for the realities of patient care in the community. She has established strong working relationships with Elder Services agencies and physician offices across the North Shore.
She is OASIS certified and an ICD-10 quality clinical documentation specialist, with deep expertise in quality care and compliance.
Keri takes the decline in care within today’s healthcare system personally. Advocacy is at the center of her work, and her mission is to ensure patients are supported, informed, and never left to navigate the system alone.
