Our goal is to reduce stress, prevent important details from falling through the cracks, and ensure you leave every appointment feeling informed, supported, and confident so you can maximize the healthcare role in your life.
The level of appointment preparation and post-visit follow-up support will depend on the package or services you choose.
Post-Visit Follow-Up can include:
Reviewing what was discussed during the appointment and explaining any new diagnoses, test orders, or treatment changes in plain language.
Creating a simple action plan with next steps, medication changes, or follow-up appointments.
Coordinating any new referrals, tests, or prescriptions with your care team.
Checking in with you afterward to answer questions, monitor how you’re doing, and address any concerns that arise.
